Except for Sacré-Coeur, which is located along the Saguenay River, all places and municipalities of the RCM are along Quebec Route 138 directly on the shores of the Saint Lawrence River.
The route continues north out of the village to the Canadian border in Constable, where it becomes Route 138 upon entering Quebec at the Trout River Border Crossing.
The administrative county seat remained in Cap-Santé while moving from the County Corporation building to a newly built one located further east on 185, Route 138 at a slight distance from the historic village of Cap-Santé.
Its northern terminus is in Saint-Roch-de-Mékinac at the junction of Route 155, and the southern terminus is at the junction of Route 138 in Sainte-Anne-de-la-Pérade.
From there, it proceeds north toward Saint-Chrysostome, from there it follows the north shore of the Rivière des Anglais until it connects with Route 138 in Très-Saint-Sacrement just northeast of the enclaved municipality of Howick.
The highway runs from the junction of Route 138 in Forestville and ends over 80 kilometers further north into large wooded areas which the main purpose of the road is for the wood industry in the Côte-Nord region.
