After designer Tom Moldvay's death in 1997, Steve Winter called The Isle of Dread "Tom's work that had the widest impact", as its inclusion in the Expert Set "made it one of the most widely known and played adventures for years".

The second version, which first appeared in 1983, was packaged with the revised version of the Expert Set by Frank Mentzer, and featured cover art by Timothy Truman.
