Middleton was born in January 1890 in Norristown, Pennsylvania, to Daniel and Anne Middleton, the second of their four children.

Middleton was an honorary Fellow of the Royal Society of Medicine (UK); a Master and former President of the American College of Physicians; a recipient of the Alumni Award of Merit from the University of Pennsylvania; and a recipient of the UW Alumni Award for Distinguished Teaching.
