The German Agency for Quality in Medicine (AEZQ) - in German "Ärztliches Zentrum für Qualität in der Medizin (ÄZQ)", established in 1995 and located in Berlin co-ordinates healthcare quality programmes with special focus on evidence-based medicine, medical guidelines, patient empowerment, patient safety programs, and quality management.
The National Patient Safety Foundation began as an idea proposed in 1996 at a large conference on medical error that was organized by the American Association for the Advancement of Science, the American Medical Association (AMA), and the Annenberg Center for Health Sciences at Eisenhower Medical Center in California and funded by The Robert Wood Johnson Foundation.
Its primary product, ScriptSwitch, is sold predominantly to NHS Primary Care Organisations who use it to provide GPs with locally authored information and advice at the point of prescribing which can contain patient safety messages, drug switch recommendations and other information.
In 2002, Virginia Mason spearheaded an effort to improve patient safety and quality of care by adopting the Toyota Production System (TPS) to health care.
